I made a guide about the Item Lab back in PSOBB CA forums. Our private server for PSOBB is dead, but it doesn't mean I can't share something that can potentially be useful. At the moment, I can only manage two. lol (example: Wolga Soul + Technique III + Spirita Boost) with 100% 100% 100% chances (using +20% success rate booster and a Spirita Boost item (which is always 100% success rate) - If you would like me to walk you through the process in game, whisper me or ask me in teamspeak. Our team leader can also explain the process for you too, and he can probably give you better advice than I can in regards to what Affixes you should probably chose. (This process involves using two "fodder" weapons + a Boost affix) Here's a couple of usefull links and an interesting screenshot of the "Extra Slot success rate list"Affix list on CirnopediaMassive amounts of info on affixing (use google chrome to translate)As you can see in this chart, if you try to do 4 slots affixing, you will not be able to manage it with a 100% success rate.
